How to Send a Case

SEND YOUR CASE
AT YOUR CONVENIENCE

We're here to support you in every step of the process. ProperGuide is proud to accept your case at your convenience. Send us a digital impression, upload it through your doctor portal, or mail in a traditional case.

Digital Impressions

Send your scan files to properguideimplant@gmail.com
We accept scans from most major scanning partners.

Doctor Portal Upload

  1. Create an account or login through our Doctor Portal.

  2. Upload relevant case files, photos, and lab instructions.

Traditional Cases

  1. Download, print, and completely fill out the Rx form.

  2. Securely package your case and Rx form and mail it to us.

Packaging and Shipping a Case Box

  1. Disinfect all case materials, place them into a bag and seal.

  2. Bubble wrap each model individually or place them in foam inserts.

  3. Insert completed Rx form and arrange all contents neatly into the box.

  4. Use bubble wrap or protective foam to fill any remaining gaps or space.

  5. Tape box securely shut and attach a shipping label.​

***Pack multiple cases in one shipment to save costs.
