Send a Case
Digital Cases

Create an account or login through our Doctor Portal.

Upload relevant case files, photos, and lab instructions.
OR
Send us your scans directly from your scanner.
Traditional Cases
-
Download, print, and completely fill out the Rx form.

Securely package your case and Rx form and mail it to us.
***For boxes and shipping labels, please email us.
Packaging and Shipping a Case Box
-
Disinfect all case materials, place them into a bag and seal.
-
Bubble wrap each model individually or place them in foam inserts.
-
Insert completed Rx form and arrange all contents neatly into the box.
-
Use bubble wrap or protective foam to fill any remaining gaps or space.
-
Tape box securely shut and attach a shipping label.
***Pack multiple cases in one shipment to save costs.
